Terms of Service

These Terms of Service govern the provision of home care services by Vitalcare Support Pty Ltd (“we”, “us”, or “our”) to you (“client”). By engaging our services, you agree to these terms.

Services

We provide Home Care Package (HCP) and Support at Home (SAH) services as agreed in your personalised Care Plan. Services are delivered in accordance with the Aged Care Quality Standards and the Charter of Aged Care Rights. All services are subject to availability and may be adjusted based on your assessed needs and available funding.

Fees and Payments

Our management fee is a flat 10% of your Home Care Package or Support at Home budget. Service fees are charged at the rates outlined in our current Pricing Schedule. Fees may be adjusted from time to time with 30 days' written notice. You are responsible for any client contributions as determined by Services Australia. Invoices are issued monthly and payment is due within 14 days.

Cancellation and Termination

You may cancel a service appointment with at least 24 hours' notice without charge. Appointments cancelled with less than 24 hours' notice may incur a cancellation fee. Either party may terminate the service agreement by providing 30 days' written notice. Upon termination, we will provide a final statement and arrange transfer of care to your new provider if applicable.

Client Contributions

Client contributions are determined by Services Australia based on your income assessment. You must notify Services Australia of any changes to your income that may affect your contribution amount. Failure to pay client contributions may affect the delivery of services.

Privacy

We handle your personal information in accordance with our Privacy Policy, the Privacy Act 1988, and the Australian Privacy Principles. By using our services, you consent to the collection, use, and disclosure of your personal information as described in our Privacy Policy.

Complaints and Feedback

We welcome feedback and take complaints seriously. You may lodge a complaint directly with us by phone, email, or in writing. If you are not satisfied with our response, you may contact the Aged Care Quality and Safety Commission on 1800 951 822 or visit agedcarequality.gov.au.

Changes to These Terms

We may update these Terms of Service from time to time. We will provide you with 30 days' written notice of any material changes. Continued use of our services after changes take effect constitutes acceptance of the updated terms.
